  • Independent Medical Examination Reports
  • Independent Record Review Reports
  • Pre-incident and Post-incident Medical Records
  • Records from Industrial Insurance, Claim Administrators and Self-insured Employers
  • Academic Transcripts
  • Employment Records
  • Military Records and VA Medical Records
  • Income Tax Returns
  • Social Security Administration
  • Disability Applications and Award Letters

By using this special Social Security Administration form, you will receive a document showing all prior earnings as well as the name and address of all prior employers. Be sure to choose Itemized Statement of Earnings on Page 2 of the form beginning with the calendar year when the plaintiff was age 18, to the present. Once this information is received you can then request employment records from any or all prior employers. Be sure to submit this form as soon as you become aware of a wage claim.

Q. How should records be sent to you?
A. Please transfer all files to us by email. Please include the name of the Plaintiff in the subject line of all emails.

Q. Is it necessary for you to personally interview the Plaintiff?
A. Although a personal interview can be helpful, it is not mandatory, as long as adequate records are provided. However, we always recommend requesting a personal interview with the plaintiff.

Q. How long will it take to prepare a report?
A. We will accommodate your schedule. Please provide us with as much advance notice as possible as our calendar is often booked several months in advance. Early referral is always the best practice.

Q. Are you able to handle cases anywhere in the United States?
A. Yes. Our methodology is effective regardless of the jurisdiction.
